Appalachian Agency for Senior Citizens is proud to announce we’ve been selected as a finalist in Toyota’s 100 Cars for Good Program. Now we need your support! Vote for AASC at www.100carsforgood.com on November 12, 2013.
Appalachian Agency for Senior Citizens (AASC) is a private non-profit organization formed in 1975. At AASC we firmly believe in fostering programs aimed at helping seniors stay within their homes and community as long as possible with dignity and grace. Our numerous programs work toward that goal, from Meals on Wheels delivery for home-bound seniors and disabled persons to homemakers and personal care aides.
If AASC were selected to receive a Toyota Prius we would utilize the vehicle to deliver meals to home-bound seniors in our four county service area which includes Buchanan, Dickenson, Russell and Tazewell counties. The Prius would also be used for the Community-based Care Transitions Program to visit Medicare beneficiaries once they are discharged from the hospital to their home. The Care Transitions Program is dedicated to improve the quality of health for Medicare beneficiaries. Our four counties are documented as having the worst health outcomes in the state of Virginia by the Weldon Cooper Study. The Care Transitions Program provides the community-based services needed to prevent readmission to the hospital and advocates for seniors to become proactive with their own health needs through education, medication management and transportation for their follow-up primary care visit.
Toyota’s 100 Cars for Good program will award 100 vehicles over the course of 50 days to 100 deserving nonprofit organizations based on votes from the public. Winning organizations will use the Toyota vehicles to help expand their reach and missions within their communities. Please support Appalachian Agency for Senior Citizens on November 12, 2013. AASC is one of Virginia’s 25 Area Agencies on Aging designated by the Virginia Department of Aging and Rehabilitative Services to carry out the department’s mission to foster the dignity, independence and security of older Virginians by promoting partnerships with communities at the local level.
